DISTURBANCES OF BRAIN SEROTONIN METABOLISM AFFECT THE BIOMECHANICAL PROPERTIES OF FEMORAL DIAPHYSIS IN RATS WITH EXPERIMENTAL CHRONIC RENAL FAILURE

Objectives. Patients with chronic kidney disease (CKD) are at risk of developing CKD-mineral bone disorder (CKD–MBD). Bone loss occurs even in early stages of CKD, and worsens as renal function declines. Decrease in bone strength, which clinical manifestation is tendency to fractures, has been recognizes as an important component of CKD-MBD. Serotonin (5-HT) is a bioamine regulating bone mass accrual depending on its site of synthesis – it decreases accrual when synthesized in the gut, and increases it when synthesized in the brain. Brain 5-HT, by binding to its receptor 5-HT2c on hypothalamic neurons, decreases sympathetic activity and favors bone mass accrual.
Aim. We used the experimental 5/6 nephrectomy model of chronic renal failure (CRF) to perform the study explaining the impact of the hypothalamic serotonin on the biomechanical properties of femoral diaphysis in rat.
Material and methods. Forty 4 weeks-old Wistar male rats were randomly allocated to sham-operation (Controls, n=16) or 5/6 nephrectomy (CRF, n=24). After one (CRF1) and three months (CRF3) of CFR development the animals were sacrificed, their femurs were excised for three-point bending test and hypothalamic tryptophan and 5-HT levels were determined by high pressure liquid chromatography (HPLC).
Results. Hypothalamic TRP concentrations were significantly lower in CRF in comparison with controls after 1 month (p<0.001) and 3 months (p<0.01) of CRF development. 5HT/TRP ratio, reflecting serotoninergic pathway activation, was 6 times higher in CRF1 than in respective controls. 5HT levels were only slightly reduced in CRF1 compared to controls (p<0.05), whereas there was no difference between this hypothalamic monoamine levels between CRF3 and respective controls. However, the age-dependent decrease in serotonin concentrations were observed in CRF as well as in controls. Yield load (Fy) has been significantly decreased, whereas work to failure (W) and yield stress (ϭy) were increased in the femurs of CRF rats compared to controls. Serotonin and 5HT/TRP ratio were inversely associated with Fy and stiffness, whereas they were positively related to displacement at fracture (dl Fu) and ultimate stress (ϭu). Moreover, 5HT was positively associated with Young modulus of elasticity (E) in rats with CRF.
Conclusions. Despite the reduced TRP concentration, the synthesis of hypothalamic serotonin is strongly activated in animals with early stage of CRF. Brain serotonin seems to improved the material properties and bone plasticity, whereas it reduces the stiffness of femoral diaphysis in rats with CRF.

Cel. Wykonano badania tkanki kostnej gąbczastej oraz warstwy podchrzęstnej głowy kości udowej pacjentów z rozpoznaniem choroby zwyrodnieniowej stawu biodrowego oraz pacjentów zdrowych. W badaniach wykorzystano metodę mikrospektroskopii Ramana. Metoda ta umożliwia określenie składu chemicznego oraz struktury tkanki kostnej na poziomie mikrostrukturalnym, czyli określenie zawartości i jakości składnika mineralnego (kryształów hydroksyapatytu i węglanoapatytu) oraz składnika organicznego (kolagenu), a także struktury konformacyjnej kolagenu.
Wnioski. Porównano zmiany w składzie i strukturze tkanki kostnej gąbczastej oraz warstwy podchrzęstnej głowy kości udowej pacjentów z koksartrozą i pacjentów zdrowych. Zmiany te dotyczyły stosunku zawartości kryształów hydroksyapatytu do kryształów węglanoapatytów i kryształów hydroksyapatytu do kolagenu oraz stosunku zawartości struktury helikalnej kolagenu do struktury kłębka statystycznego kolagenu. W warstwie podchrzęstnej kości zawartość hydroksyapatytu w porównaniu do zawartości węglanoapatytu oraz hydroksyapatytu w porównaniu do zawartości kolagenu, jest mniejsza u pacjentów chorych niż u pacjentów zdrowych. Zaobserwowano, że u pacjentów zdrowych w porównaniu do pacjentów z koksartozą, w warstwie podchrzęstnej kości częściej występuje struktura uporządkowanej alfa helisy niż nieuporządkowana struktura kłębka statystycznego. Stwierdzono, że w przypadku tkanki kostnej gąbczastej z obszaru przy warstwie podchrzęstnej kości nie występują opisane wyżej różnice pomiędzy tkanką pacjentów chorych i pacjentów zdrowych, co jest ważnym rezultatem.

RAMAN MICROSPECTROSCOPY STUDY OF BONE TISSUE OF THE FEMORAL HEAD OF PATIENTS WITH DEGENERATIVE DISEASE OF THE HIP JOINT

Aim. Spongy and subchondral bone tissues from the femoral head of patients diagnosed with degenerative disease of the hip joint and healthy subjects were studied by the method of Raman microspectroscopy. The method permits determination of chemical composition and structure of the bone tissue at a microstructural level, i.e. the content and quality of the mineral component (hydroxylapatite and carbonapatite crystals) and the organic component (collagen) as well as the conformation of collagen.
Conclusions. Differences in the composition and structure of the above bone tissues from the femoral head in the patients diagnosed with coxarthrosis and healthy subjects were detected and compared. The differences were in the ratios of the content of hydroxylapatite to carbonapatite crystals and to collagen as well as the ratio of the content of collagen in the helical structure and random coil structure. In the patients diagnosed with coxarthrosis, the content of hydroxyapatite to that of carbonapatite and the content of hydroxyapatite to that of collagen are lower than in the healthy subjects. In the healthy subjects the subchondral layer of bone tissue shows higher content of collagen in the ordered alpha helix structure than that in the disordered structure of random coil. In the spongy bone tissue from the layer close to the subchondral bone tissue no differences were observed between the healthy subjects and patients diagnosed with coxarthrosis, which is an important finding.

ZALEŻNOŚĆ STĘŻEŃ WYBRANYCH CYTOKIN Z OBROTEM KOSTNYM I ZMIANAMI BMD W AKROMEGALII

Akromegalii towarzyszą zmiany gęstości kości i aktywności przebudowy kości. Niektóre cytokiny uczestniczą w procesie przebudowy kości, a ich stężenia obwodowe odzwierciedlają aktywność obrotu kostnego. Przebadano stężenia w osoczu IL-1β IL-6, TNF-α i TGF-β u 20 chorych na czynną akromegalię w porównaniu do 20 wyleczonych i 10 zdrowych osób z grupy kontrolnej Stężenia cytokin były odnoszone do poziomów GH, IGF-1, osteokalcyny (OC) i produktów degradacji C-końcowych telopeptydów kolagenu typu 1 – CrossLaps (CL). Badanie densytometryczne kręgosłupa lędźwiowego, szyjki kości udowej i przedramienia wykonano metodą DEXA. Stężenia IL-6, OC i CL były wyższe (p <0.05) u chorych z czynną akromegalią niż u wyleczonych i zdrowych osób. Stwierdzono dodatnie korelacje między IL-1β i CL (r = 0.34; p < 0.05); TNF-α i OC (r = 0.58; p < 0.05) w czynnej akromegalii. Poziom IL-6 korelował z GH (r = 0.3; p< 0.05), CL z GH (r = 0.3; p < 0.05), OC z GH (r = 0.3; p < 0.05), OC z IGF-1 (r = 0.34; p < 0.05) w całej grupie pacjentów z czynną i wyleczoną akromegalią. Poziom TGF-β korelował ujemnie z wartościami BMD szyjki kości udowej, trójkąta Warda i krętarza (p<0.05) u chorych na czynną akromegalię, ale IL-6 korelował dodatnio z BMD trójkąta Warda i krętarza (p < 0.05), a TNF-α korelował z BMD kręgosłupa lędżwiowego i przedramienia (p < 0.05) w całej grupie pacjentów. Ponadto, wartości densytometryczne w szyjce kości udowej, trójkącie Warda i krętarzu były statystycznie istotnie zależne od maksymalnych wartości GH i IGF-1. TNF-α i IL-1β odzwierciedlają aktywność obrotu kostnego w czynnej akromegalii. TGF-β TNF-α i IL-6 biorą udział w zmianach BMD w akromegalii.

SOME CIRCULATING CYTOKINES ARE CORRELATED WITH BONE TURNOVER AND BMD CHANGES IN ACROMEGALY

Acromegaly is associated with changes in BMD and in the activity of bone turnover. Some cytokines are involved in the bone remodeling process, and their circulating levels reflect the activity of bone turnover. We have studied serum IL-1B, IL-6, TNF- B and TGF- B concentrations in 20 patients with active acromegaly in comparison to 20 cured patients and 10 controls. Cytokine levels were related to GH, IGF-1, osteocalcin (OC) and degradation products of C-terminal telopeptides of type 1 collagen – Cross- Laps (CL). Densitometry of the lumbar spine, femoral neck and forearm was carried out using the DEXA method.
Serum IL-6, OC and CL levels were higher (p<0.05) in patients with active disease in comparison to cured patients and controls. There were positive correlations between IL-1 B and CL (r = 0.34; p<0.05); TNF- B and OC (r = 0.58; p<0.05) in active acromegaly. IL-6 correlated with GH (r = 0.3; p<0.05), CL with GH (r = 0.3; p<0.05), OC with GH (r = 0.3; p<0.05), OC with IGF-1 (r = 0.34; p< 0.05) in the entire group of patients with active and cured acromegaly.
TGF-B correlated negatively with values of BMD within the femoral neck, Ward’s triangle and trochanter (p<0.05) in patients with active acromegaly. IL-6 was positively correlated with BMD of Ward’s triangle and trochanter (p<0.05). TNF-B revealed positive correlations with BMD of the lumbar spine and forearm (p < 0.05) in the entire group of patients. Moreover, densitometric values within the femoral neck, Ward’s triangle and trochanter were statistically significantly positively correlated with maximal basal GH and IGF-1 concentrations.
TNF-B and IL-1B reflect activity of bone turnover in active acromegaly.
TGF-B. TNF-B and IL-6 are involved in BMD changes in the disease.

Physiotherapeutic methods are: mechanotherapy, electrotherapy, magnetotherapy, termotherapy, IR, paraffinotherapy, cryotherapy, phototherapy, aerosolotherapy, climatotherapy, balneotherapy, health resort treatment and rehabilitation.
The methods of physiotherapy applied in ostheoporosis treatment are: cryotherapy, magnetotherapy, phytotherapy, crenotherapy, spa baths, ionthophoresis, electrotherapy, laserotherapy, massages, kinetic therapy – treatment by the means of movement. All these methods aim to reduce pain, improve tissue nutrition, anti-inflammatory effect, muscles relaxation, getting stronger bones and articles joints.

FEATURES OF BONE MINERALIZATION AFTER PLASTICS OF METADIAPHYSEAL DEFECTS WITH CERAMIC OSTEOAPATITE

Introduction, aim of the study
The data on actual mineral and organic contents in the bones other than injured are practically unavailable in the modern sources. Our study therefore is aimed at investigation of bone minerals in hipbone and parietal bone after experimental operations for plastics of bone defects with ceramic hydroxyapatite (OC015).
Materials, methods
126 non-linear rats with the initial mass of 135-145 grams were operated on for through perforation of both tibiae thus modeling a routine bone tissue removal. The 2.2-mm. defects in half of the total number of the animals were filled with osteoapatite granules of the respective diameter while in the rest the defects remained unfilled. The observation terms were 7, 15, 30, 60, 90 and 180 days. Upon expiration of each term the required bones were excised for further analysis.
Results and discussion
The first group showed increase of water contents in the hipbone beginning from the 7th day of observation; the values exceeded those of controls by 5.75%, 7.59%, 9.76%, 12.12%, 20.14% and 7,07% (p<0,05) respective to observation terms. The minerals were on the contrary lower than in control group in the period from the 7th to the 90th day and organic substances decreased only by the 90th day – by 12.55%. Parietal bone featured the same changes yet beginning from the 15th day of observation.
The second group exhibited actually the similar dynamics as the first group yet in the period from the 7th to the 90th days deviations amplitude was somewhat higher. In the period from the 60th to the 90th day deviations amplitude smoothed and finally by the 180th day no significant differences between two groups were observed.
Conclusions
Regeneration processes in tibiae lead to minerals and organic substances loss vs. water share increase in hipbone and parietal bone. Promoted regeneration still features similar changes yet more expressed in early terms but finally leveling to baseline in later terms.

KURKUMINA STOSOWANA W WYSOKIEJ DAWCE KORZYSTNIE WPŁYWA NA WŁAŚCIWOŚCI MECHANICZNE KOŚCI U SZCZURÓW Z NIEDOBOREM ANDROGENÓW

Wstęp. Kurkumina jest polifenolem występującym w kurkumie, sproszkowanym kłączu ostryżu długiego (Curcuma longa L.). Kurkuma stosowana jest od wieków w tradycyjnej medycynie indyjskiej. Kurkumina moduluje aktywność czynników transkrypcyjnych, kinaz białkowych i innych enzymów, cytokin, czynników wzrostu, receptorów i in., z których wiele bierze w regulacji metabolizmu kostnego. Szczególne znaczenie może mieć oddziaływanie kurkuminy na ścieżki sygnałowe zależne od RANKL (w tym na NF-κB) w komórkach osteoklastycznych oraz ścieżkę sygnałową Wnt w komórkach osteoblastycznych. Do działania kurkuminy na układ kostny przyczyniać się może jej działanie antyoksydacyjne. Dotychczas brak jest jakichkolwiek danych na temat działania kurkuminy na układ kostny u ludzi, jednak w warunkach eksperymentalnych wykazano, że kurkumina może hamować rozwój osteoporozy wywołanej niedoborem estrogenów.
Cel. Celem niniejszej pracy było zbadanie wpływu kurkuminy na właściwości mechaniczne kości u szczurów z niedoborem androgenów wywołanym przez orchidektomię.
Materiał i metody. Badania przeprowadzono na dojrzałych szczurach samcach szczepu Wistar, podzielonych na 4 grupy (n=10): I – szczury kontrolne nieorchidektomizowane, II – szczury kontrolne orchidektomizowane, III – szczury orchidektomizowane, którym podawano kurkuminę w dawce 10 mg/kg p.o. dziennie, IV – szczury orchidektomizowane, którym podawano kurkuminę w dawce 100 mg/kg p.o. dziennie. Zabieg orchidektomii został przeprowadzony w znieczuleniu ogólnym wywołanym ketaminą z ksylazyną u 3-miesięcznych szczurów. Stosowanie kurkuminy rozpoczęto po 4 tygodniach od zabiegu. Kurkuminę podawano sondą dożołądkową, raz dziennie, przez 4 tygodnie. Szczury kontrolne otrzymywały wodę w tej samej objętości (2 ml/kg p.o.). Po 8 tygodniach od orchidektomii zwierzęta uśmiercano. W surowicy oznaczano stężenia markerów metabolizmu kostnego: C-końcowych fragmentów kolagenu typu I uwalnianych podczas resorpcji kości i osteokalcyny. Określano masę prostaty i grasicy, masę i parametry geometryczne kości udowej i piszczelowej oraz badano właściwości mechaniczne trzonu kości udowej (kość zbita) i przynasady bliższej kości piszczelowej (kość gąbczasta) w testach trzypunktowego zginania. Ponadto badano wytrzymałość mechaniczną szyjki kości udowej, zbudowanej z kości zbitej i gąbczastej, z zastosowaniem testu kompresyjnego. Badania wytrzymałościowe przeprowadzono przy użyciu aparatu Instron 3342 500N.
Wyniki. Niedobór androgenów spowodował statystycznie istotne zmniejszenie masy prostaty i zwiększenie masy grasicy. Masa badanych kości uległa zmniejszeniu, resorpcja kości nieznacznie nasiliła się, a właściwości mechaniczne trzonu kości udowej uległy pogorszeniu. Nie stwierdzono statystycznie istotnych zmian właściwości mechanicznych kości gąbczastej.
Podawanie kurkuminy w obydwu dawkach u szczurów z niedoborem androgenów nie wpływało na masę narządów androgenozależnych oraz masę i parametry geometryczne kości. Jednak po stosowaniu kurkuminy w dawce niższej zwiększał się poziom markerów obrotu kostnego i występowała tendencja do poprawy wytrzymałości kości udowej, a w dawce wyższej – istotnie poprawiała się wytrzymałość trzonu kości udowej oraz przynasady kości piszczelowej w porównaniu do zwierząt kontrolnych orchidektomizowanych.
Wnioski. Podsumowując, kurkumina stosowana w wysokiej dawce wywierała korzystny wpływ na układ kostny orchidektomizowanych szczurów, prawdopodobnie działając przez mechanizmy różne od androgenowych.

HIGH-DOSE CURCUMIN FAVORABLY AFFECTS BONE MECHANICAL PROPERTIES IN RATS WITH ANDROGEN DEFICIENCY

Objectives. Curcumin is a polyphenol present in turmeric, dried ground rhizomes of the turmeric plant (Curcuma longa L.). Turmeric has been used for centuries in the traditional Indian medicine. Curcumin modulates the activity of transcription factors, protein kinases and other enzymes, cytokines, growth factors, receptors and others, many of which take part in regulation of bone metabolism. Of particular importance may be curcumin effects on the pathways dependent on RANKL signaling (including NF-κB) in osteoclastic cells, and Wnt signaling pathway in osteoblastic cells. Antioxidant properties of curcumin may contribute to its skeletal effects. So far there are no data on the effects of curcumin on the skeletal system in humans, but, in experimental conditions, curcumin inhibited development of osteoporosis induced by estrogen deficiency.
Aim. The aim of the present study was to investigate the effect of curcumin on bone mechanical properties in rats with androgen deficiency induced by orchidectomy.
Materials and methods. The experiments were carried out on mature male Wistar rats, divided into 4 groups (n = 10): I – Non-orchidectomized control rats II – Orchidectomized control rats, III – Orchidectomized rats treated with curcumin (10 mg/kg p.o. daily), IV – Orchidectomized rats treated with curcumin (100 mg/kg p.o. daily). Orchidectomy was performed in 3-month-old rats, under ketamine-xylazine anesthesia. Administration of curcumin started 4 weeks after the surgery. Curcumin was administered, by a stomach tube, once daily for 4 weeks. Control rats received water at the same volume (2 ml/kg p.o.). At 8 weeks after the orchidectomy, the animals were sacrificed. In the serum, markers of bone metabolism: C-terminal fragments of type I collagen released during bone resorption and osteocalcin were determined. The prostate and thymus mass, mass and geometrical parameters of the femur and tibia, and mechanical properties of the femoral diaphysis (compact bone) and tibial proximal metaphysis (cancellous bone), in three-point bending tests, were examined. Moreover, mechanical strength of the femoral neck (built of cortical and cancellous bone) was tested using a compression test. The mechanical tests were performed using an Instron 3342 500N apparatus.
Results. Androgen deficiency resulted in a statistically significant decrease in the prostate mass and an increase in the thymus mass. The mass of the bones decreased, bone resorption slightly intensified and mechanical properties of the femoral diaphysis worsened. There were no statistically significant changes in the mechanical properties of cancellous bone.
Administration of curcumin at both doses to androgen-deficient rats did not affect the mass of androgen-dependent organs, and bone mass and geometrical parameters. However, after administration of curcumin at the lower dose, bone turnover markers increased and there was a tendency to improve the femoral diaphysis strength, and, after the higher dose, there was a significant increase in the strength of the femoral diaphysis and tibial metaphysis in comparison with the orchidectomized control animals.
Conclusions. In conclusion, curcumin used at a high dose beneficially affected the skeletal system of orchidectomized rats, probably acting through mechanisms different from androgenic ones.

Aim. to study the fractal properties of spongy bone of human sacral vertebrae, to determine the value of its fractal dimension.
Materials and methods. 74 human preparations sacrum were used in the work. We took samples of spongy bone in the following areas: two blocks from the bodies of SI – SIII and one block from the bodies of SIV – SV. Then each of the blocks were photographed in various projections using Videopresenter Svp-5500. Next, we determined fractal dimension of samples by the method of splitting into squares using IMAGEJ 1.20s. The data were processed using Sheffe’s method of multiple comparisons.
Results. The average fractal dimension of spongy tissue is 1.70. Average valua of dimension in SI – SIII vertebrae is 1.82, in vertebrae SIV – SV – 1.33. Given that this difference is statistically reliable, we can speak about the tendency of decrease of fractal dimension in the direction from SI to SV.
Conclusions. The tendency to reduce the fractal dimension of spongy tissue bodies sacral vertebrae from SI to SV. This may indicate a facilitation of spatial organization of spongy bone tissue in relation to the reduction of mechanical load on the sacrum in this direction.

Cel: Ocena metabolizmu Ca-P-Mg i BMD u chorych z zapaleniem przełyku(ZP) leczonych przez rok omeprazolem(OM). Metoda: Kryteria włączenia: co najmniej nadżerkowe ZP w endoskopii; wiek 18-80 lat dla mężczyzn i 60-80 dla kobiet; bez chorób i leków wpływających na metabolizm Ca-P-Mg. OM, 20 mg/d, podawano przez rok, część chorych zakażonych H. pylori(Hp) na początku eradykowano. Oznaczano Ca, P, Mg, całkowitą fosfatazę alkaliczną(AP) w surowicy i Ca w moczu (FUCa/Kr) przed, po 6 i 12 miesiącach OM; osteokalcynę(OC) w osoczu i dezoksypirydynolinę(dP) w moczu przed i po 6 miesiącach; BMD(L2-4, Neck i Total) metodą DEXA przed i po roku. Zakażenie i eradykację Hp potwierdzano testem ureazowym. Testy t-Studenta, Manna-Whitney’a i Fishera, ANOVA, regresja wieloczynnikowa służyły do analizy danych. Wyniki: Badania ukończyło 30 chorych(20 mężczyzn, 10 kobiet) w wieku 58,714,0 lat. Po 6 miesiącach OM wzrosła OC w osoczu w całej grupie z 4,62±3,66 do 8,15±6,20 ng/ml(p<0,05), a po roku AP w surowicy u mężczyzn z 71,9±19,3 do 81,5±22,8 IU/l(p<0,05). Stężenie Ca, P i Mg w surowicy, FUCa/Kr i wydalanie dP w moczu nie zmieniło się. BMD Total wzrosła po OM z 1,106±0,084 do 1,121±0,091 g/cm2 (p<0,05) w całej grupie i u mężczyzn z 1,122±0,076 do 1,145±0,082 g/cm2 (p<0,05). BMD L2-4 i Neck nie zmieniła się. U chorych wyleczonych z zakażenia Hp spadło stężenie Ca w surowicy z 9,68±0,30 do 8,88±0,55 po 6(p<0,01) i do 9,11±0,45 mg/dl po 12 miesiącach OM (p<0,05), a wzrosła BMD Total z 1,183±0,062 do 1,214±0,069 g/cm2 (p<0,01). Wnioski: 1. Roczne leczenie OM nie zmienia Ca, P, Mg we krwi; 2. OM korzystnie modyfikuje tworzenie i resorpcję kości u mężczyzn, ale nie wpływa na przebudowę kości u kobiet po 60 r. ż.; 3. Eradykacja Hp może mieć korzystny wpływ na metabolizm mineralny.

Ca-P-Ma METABOLISM AND BONE MINERAL DENSITY (BMD) IN PATIENTS WITH OESOPHAGITIS TREATED WITH OMEPRAZOLE

Aim: To assess Ca-P-Mg metabolism and BMD in patients with oesophagitis(OE) treated for 1yr with omeprazole(OM).
Methods: Inclusion criteria: at least erosive OE in endoscopy; age 18-80 yrs for males and 60-80yrs for females; lack of disease and drugs influencing Ca-P-Mg metabolism. OM 20 mg/d, was given for 1 yr, a portion of H. pylori(Hp) infected patients underwent eradication at the beginning. Serum Ca, P, Mg, total alkaline phosphatase(AP) and urinary Ca(FUCa/Cr) before, after 6 and 12 months of OM; plasma osteocalcin (OC) and urinary deoxypyridinoline (dP) before and after 6 months; BMD(L2 -4, Neck and Total) by DEXA before and after 1 yr were determined. Hp infection and eradication were assessed by a urease test. For
data analysis the Student’s t-test, Mann-Whitney’s and Fisher’s tests, ANOVA, multiple regression were applied.
Results: 30 patients (M 20, F 10) aged 58.7±14.0 yr completed the study. After 6 months of OM plasma OC increased in the whole group from 4.62± 3.66 to 8.15±6.20 ng/ml (p<0.05) and after 1 yr serum AP did so in males from 71.9±19.3 to 81.5±22.8 lU/l (p<0.05). Serum Ca, P and Mg, FUCa/Crand urinary dP did not change. Total BMD increased after OM in the whole group from 1.106±0.08 to 1.121±0.09 g/cm2 (p<0.05) and in males from 1.122±0.08 to 1.145±0.08 g/cm2 (p<0.05). BMD at L2-4 and Neck did not change. In Hp eradicated patients serum Ca decreased from 9.68±0.30 to 8.88±0.55 after 6 (p<0.01) and to 9.11±0.45 mg/dl after 12 months of OM (p<0.05), and Total BMD increased from 1.183±0.062 to 1.214±0.069 g/cm2 (p<0.01).
Conclusions: 1.1-yr OM therapy does not change serum Ca, P and Mg. 2. OM favorably modifies bone formation and resorption in men, but does not effect bone remodeling in women aged above 60 yrs. 3. Hp eradication may have a favorable effect on mineral metabolism.

METODY WYZNACZANIA ANIZOTROPII KOŚCI BELECZKOWEJ NA PODSTAWIE OBRAZÓW W SKALI SZAROŚCI

Wstęp
Mechaniczne i strukturalne własności kości beleczkowej są anizotropowe, t.j. zależą od kierunku, w którym są wyznaczane. Parametry opisujące anizotropię nie korelują z gęstością kości beleczkowej, mogą więc być potencjalnie użyte do poprawy skuteczności oceny ryzyka złamań osteoporotycznych. Anizotropia kości beleczkowej jest typowo mierzona na obrazach binarnych. Jednakże proces akwizycji obrazu kości beleczkowej, przeprowadzony w warunkach in vivo narzuca ograniczenia na dostępną ilość informacji o architekturze kości beleczkowej. Segmentacja prowadzi do dalszej degradacji jakości tej informacji.
Cel pracy
Chociaż metody analizy orientacji tekstur zapisanych w pełnej skali szarości są standardowym narzędziem analizy obrazów, to nie były dotychczas stosowane do ilościowego opisu anizotropii kości beleczkowej na podstawie danych otrzymanych w warunkach pomiaru in vivo. W pracy przedstawiono przykłady analizy orientacji beleczek kostnych na podstawie obrazów w skali szarości.
Materiały i metody
Osiowe szlify kości beleczkowej o grubości 200m otrzymano z kręgów lędźwiowych L3 16 mężczyzn. Przy pomocy kamery CCD sprzężonej z mikroskopem uzyskano obrazy wysokiej rozdzielczości preparatów. Stworzono program do symulacji procesu akwizycji obrazów w pomiarze tomograficznym. Program ten użyto do otrzymania obrazów niskiej rozdzielczości preparatów.
Anizotropię mierzono na obrazach niskiej i wysokiej rozdzielczości przy użyciu metod: mean-intercept length (MIL), volume orientation (VO), gray-level structure tensor (GST) oraz interaction maps (IM). Wyznaczono główny kierunek anizotropii i współczynnik anizotropii. W celu oceny powtarzalności pomiarów parametrów anizotropii wyliczono również standaryzowany współczynnik zmienności StdCV.
Wyniki
Zbadano korelację pomiędzy wynikami otrzymanymi dla obrazów niskiej i wysokiej rozdzielczości dla różnych poziomów szumu. Współczynnik korelacji dla współczynika anizotropii wynosił 62%, 62%, 87% oraz 6% dla metod MIL, VO, GST i IM. Współczynnik korelacji dla głównego kierunku anizotropii wynosił 81%, 94%, 90% oraz 75% dla metod MIL, VO, GST i IM. Współczynnik zmienności pomiaru współczynnika anizotropii wyznaczonego na obrazach niskiej rozdzielczości wynosił 210%, 66%, 38% i 72% dla MIL, VO, GST i IM. Współczynnik zmienności pomiaru głównego kierunku anizotropii wyznaczonego na obrazach niskiej rozdzielczości wynosił 122%, 66%, 43% oraz 45% dla MIL, VO, GST i IM.
Wnioski
W pracy pokazano, że pomiary anizotropii przeprowadzone na obrazach niskiej rozdzielczości zapisane w pełnej 256 stopniowej skali szarości dają zasadniczo lepsze wyniki niż pomiary przeprowadzone w oparciu o obrazy binarne. Najlepsze wyniki otrzymano dla metody opartej na pomiarze gradientu intensywności. Słaba powtarzalność i słaba korelacja z danymi uzyskanymi dla obrazów wysokiej rozdzielczości ograniczają zastosowanie metod opartych na obrazach binarnych do analizy anizotropii kości beleczkowej.

QUANTIFYING TRABECULAR BONE ANISOTROPY FROM GREY-LEVEL IMAGES

Introduction
Mechanical and structural properties of trabecular bone are anisotropic, i.e. they depend on the test direction. The anisotropy of trabecular bone structure provides information, which is independent of bone density and may be used to improve evaluation of bone strength. Numerous methods of quantifying anisotropy have been introduced and used to the analysis of only segmented images of trabecular bone. However degradation of image if the process of in vivo acquisition followed by segmentation may lead to serious reduction of the diagnostic power of architectural parameters. For example it is known that estimates of histomorphometric measures based on low-resolution segmented images of trabecular bone correlate only mildly with estimates based on high-resolution images. It would be thus plausible to derive structural information should directly from grey-level images.
Aim of the work
Although methods of analysis of texture orientation exist in image processing science, there was only little effort to apply them to the field of trabecular bone research. We discuss possible approaches to the analysis of anisotropy of trabecular bone directly from gray-level images.
Materials and methods
Axial trabecular bone sections with thickness of 200m were obtained from L3 vertebral bodies of 16 men. High-resolution images of the specimens were acquired using CCD camera connected to a microscope. A process of CT examination was simulated and low-resolution images of the sections were obtained.
Anisotropy of both low- and high-resolution images was computed using methods based on mean-intercept length (MIL), volume orientation (VO), gray-level structure tensor (GST) and interaction maps (IM). Main anisotropy direction and anisotropy ratio were computed. Reproducibility (standardized coefficient of variation StdCV) of the anisotropy measures was also estimated.
Results
We examined correlation between the low-and high-resolution estimates of anisotropy measures for signal-to-noise ratio ranging from 1 to 2 to reproduce in vivo acquisition circumstances. The correlation coefficient between low- and high resolution estimates of anisotropy ratio was equal to 62%, 62%, 87% and 6% for MIL, VO, GST and IM, respectively. The correlation coefficient between low- and high resolution estimates of the main anisotropy direction was equal to 81%, 94%, 90% and 75% for MIL, VO, GST and IM, respectively. Reproducibility of low-resolution estimate of anisotropy ratio was equal to 210%, 66%, 38% and 72% for MIL, VO, GST and IM, respectively. Reproducibility of low-resolution estimate of the main anisotropy direction was equal to 122%, 66%, 43% and 45% for MIL, VO, GST and IM, respectively.
Conclusion
It has been shown in the study that gray-level-based description of anisotropy is substantially better then binary image-based description. We have found that gradient of intensity describes best orientation of trabeculae. The application of binary image-based methods to the description of trabecular anisotropy is limited because of poor reproducibility and mild correlation with high-resolution data.

USAGE PROSPECTS OF BIOLOGICAL HYDROXYAPATITE CERAMICS, SATURATED WITH MICROELEMENTS, FOR EXTREMITY BONE DEFECTS PLASTICS UNDER CONDITIONS OF REPEATED OSTEOPOROSIS

Introduction, aim of the research
The problem of choosing the material for bone defects substitution remains relevant under conditions of repeated osteoporosis, which arises after surgical ablation of benign tumours, tumor-like and dysplastic diseases. The usage of synthetic hydroxyapatite ceramics with this aim attracts special interest today.
We have analyzed the results of clinical usage of compositional material of biological origin on basis of hydroxyapatite, which was made in Ukraine and was called “ceramic osteoapatite”- CO.
Material, methods
In experiments on rats and in clinic the peculiarities of rebuilding CO-100 in the form of powder, particle size 100 mkm, pore-granular CO-015-5000 with particle size 1000-5000 mkm, pore size 90-600 mkm and OC-1, 5 in the form of blocks, saturated with microelements Cu, Zn, Se, have been studied. When using CO, saturated with Cu, Zn, the conditions for improving the repair regeneration in the place of ceramoplastics are made. Selenium combinations have an important anticarcinogenic influence on the neoplasm development, due to its antioxidant and immune response modulating characteristics. When filling in the defects of CO bone, saturated with selenium, some conditions of anticarcinogenic effect, from the side of implant, are made, thus, the probability of illness relapse is reduced and that is especially relevant in treating giant cell tumour.
The clinical part of the work includes the observations among 86 patients, in which defects plastics was performed with one of CO types, saturated with microelements, after surgery of benign tumours, tumour-like and dysplastic ailments of extremity bones.
The patients were from 7 till 52 years of age. Children aged until 15 years old were 25,6%. The majority were men (69-80,2%). Solitary bone cyst was observed in 17, aneurismal – in 4, fibrous dysplasia – in 16, enchondrome – in 19, orteochondrome – in 11, osteoma – in 4, and giant cell tumour in 15 cases.
Depending on the data, parietal and intracavitary resection was applied. In one case segmental resection was carried out at the lesion of distal radius part. The defect was removed with pore – granular CO-015, which at hardening was adapted to the form of the defect. If bone stability suffered insignificantly firm low porous block of CO-1, 5, which was placed along load axis, was applied and the rest of the cavity around the block was filled with pore granular CO-015.
Results, conclusions
Remote results were traced in 90,7% of patients. There were no signs of inflammation and implant rejection. Only one patient with giant cell tumour developed disease recurrence, which required repeated surgery. Biodegradation assessment of CO implants was conducted according to 4-points scale.
The application of different forms of CO, saturated with microelements, enables to obtain good results of treatment in the majority of patients. And namely with the study of these types of implants we connect perspectives of reconstructive orthopaedy of bone defects after operative treatment of benign tumours, tumour like and dysplastic ailments of extremities bone.

NRF2 TRANSCRIPTION FACTOR AND HEME OXYGENASE-1 AFFECTS OSTEOCLASTS DIFFERENTIATION

Objectives. Due to increased activity and number of osteoclasts (OCLs) there is a progressive demineralization/resorption of bone tissue in diseases associated with inflammation (rheumatoid arthritis) or bone metabolic diseases (osteoporosis). Acting as strong cytoprotectants and antioxidants Nrf2 transcription factor and heme oxygenase-1 (HO-1), its downstream target, have been suggested to play a role in osteoclastogenesis. Nevertheless, the studies investigating the effect of genetic manipulation of those mediators are lacking.
Aim. We aimed to investigate the effect of Nrf2 and HO-1 deficiency on OCL differentiation from bone marrow (BM) cells and specifically from bone marrow macrophages (BMMs).
Materials and methods. As a model Nrf2- or HO-1-knockout mice (Nrf2-/- or HO-1-/-, respectively) and murine monocytic cell line, RAW264.7 were used in combination with pharmacological inducers of HO-1, cobalt protoporphirin IX (CoPPIX) or hemin, as well as siRNA against HO-1.
Results. To test the effect of the absence of Nrf2 and HO-1 in BM cells on osteoclastogenesis, BM cells from Nrf2-/-, HO-1-/- and wild type (WT) mice were counted, seeded in the presence of macrophage colony-stimulating factor (M-CSF) to stimulate macrophage growth (day 0 -> day 6) and with receptor activator of nuclear factor kB ligand (RANKL) to induce osteoclasts differentiation (day 3-> day 6). Macrophage phenotype of cells at day 3 was confirmed irrespective of the genotype. Mature OCLs were identified at day 6 by tartrate-resistant acid phosphatase (TRAP) detection. Nrf2 deficiency resulted in higher number of OCLs. Reversely, after Nrf2 activation by sulforaphane in Nrf2+/+ cells no TRAP-positive signal was observed, confirming inhibitory effect of Nrf2. In addition, osteoclasts-specific genes, such as cathepsin K and integrin β3, were upregulated in Nrf2-/- BM cells (vs. Nrf2+/+). In contrast, the lack of HO-1 diminished the number of TRAP+ cells and expression of OCL markers in the population of M-CSF and RANKL-treated BM cells (vs. HO-1+/+). To confirm that the observed effects are not related to the effect of Nrf2/HO-1 on macrophage abundance, BMMs were harvested and plated in equal numbers for all genotypes. Indeed, BMM Nrf2-/- stimulation with RANKL led to a high increase in TRAP-positive cells. BMMs isolated from HO-1-/- mice formed less TRAP+ cells.
In RAW264.7 cells, CoPPIX and hemin both inhibited NFATc-1 and the transfection with siRNA against HO-1 caused similar effect. Therefore, the combination of siRNA HO-1 with CoPP IX was examined. CoPP IX prestimulation resulted in diminishment of NFATc-1 level, while additional siRNA HO-1 transfection tended to reverse this effect- enhanced the expression of NFATc-1.
Conclusions. In summary, Nrf2 deficiency exerts stimulatory effect on osteoclastogenesis, while the lack of HO-1 in BM cells or its silencing in RAW264.7 cells by siRNA decrease this process. On the other hand, HO-1 enhancement by pharmacological inducers also diminished the formation of OCLs, which could be reversed by HO-1 silencing. Those results suggest that HO-1 has to be present at specific level for proper process of osteoclastogenesis.

OBRAZ JAŁOWEJ MARTWICY GŁOWY KOŚCI UDOWEJ W MIKROSPEKTROSKOPII RAMANA

Wstęp. Najczęstszym powikłaniem leczenia złamań szyjki kości udowej z zachowaniem głowy kości udowej jest jałowa martwica głowy kości udowej. Jest ona efektem uszkodzenia naczyń doprowadzających krew do głowy kości udowej (najczęściej gałęzi wstępującej tętnicy okalającej udo przyśrodkowej).
Cel. Celem badań była ocena zmian, jakie zachodzą w przewlekle niedokrwionej tkance kostnej głowy kości udowej.
Materiał i metody. Badaniom poddano gąbczastą tkankę kostną z głowy kości udowej 5 chorych u których usunięto głowę bezpośrednio po złamaniu szyjki kości udowej i 5 chorych po 9 miesiącach od złamania po leczeniu zespoleniem, u których wystąpiła jałowa martwica głowy kości udowej. W badaniach wykorzystano metodę mikrospektroskopii Ramana, która umożliwia określenie składu chemicznego oraz struktury tkanki kostnej na poziomie mikrostrukturalnym. Badania wykonano przy pomocy konfokalnego mikroskopu ramanowskiego in Via firmy Renishaw.
Wyniki. Porównano skład i strukturę tkanki kostnej, stosunek ilości hydroksyapatytu do ilości węglanoapatytu, ilości hydroksyapatytu do ilości kolagenu oraz struktury helikalnej kolagenu do struktury statystycznego kolagenu. W tkance kostnej gąbczastej stosunek ilości węglanoapatytu do hydroksyapatytu oraz hydroksyapatytu do kolagenu jest większy u osób z jałową martwicą głowy kości udowej niż u osób bezpośrednio po złamaniu.
Zaobserwowano, iż u chorych z jałową martwicą głowy kości udowej w porównaniu do pacjentów po złamaniu osteoporotycznym stosunek ilości struktury uporządkowanej alfa helisy do nieuporządkowanej struktury kłębka statystycznego nie zmienia się.
Na podstawie wyników badań tkanki kostnej martwiczo zmienionej głowy kości udowej z zastosowaniem metody mikrospektroskopii Ramana można zaobserwować zmiany ilościowe i jakościowe składników mineralnych i organicznych w strukturze międzykomórkowej tkanki kostnej gąbczastej.
Wnioski. Dalsze badania pozwolą wykluczyć lub potwierdzić teorię niedokrwienną powstawania zmian zwyrodnieniowych stawu biodrowego. Metoda może być zastosowana jako narzędzie diagnostyczne w rozpoznawaniu i leczeniu jałowej martwicy głowy kości udowej.

ASEPTIC NECROSIS OF THE FEMORAL HEAD BY RAMAN MICROSPECTROSCOPY

Objectives. The most common complication after surgical treatment of proximal extremity of femur’s fracture, with rescuing of it’s head, is aseptic necrosis of the femur’s head. It is a direct effect of rupture of blood vessels supporting a head of femur (most commonly ascending branch of medial circumflex femoral artery).
Aim. The purpose of this study is determination of the changes in the chronic ischaemic bone tissue of the head of femur.
Materials and methods. The research was composed of samples of head of femur’s spongy bone tissue taken from 5 patients, in which the head was removed directly after fracture of the neck of femur. Another 5 patients were sampled in 9 months after fracture, which had been treated by several methods of fixation.
The Raman’s microspectroscopy technique was performed, which enables to estimate the chemical composition and structure of bone tissue on microstructural level. The Raman’s confocal microscope “in Via” made by Renishaw company was used for spectroscopic measurements.
Results. The composition and structure were compared, the ratio of hydroxyapatite to carboxyapatite, the amount of hydroxyapatite to collagen and helical structure of collagen to the structure of typical collagen.
In spongy bone tissue, the ratio of carboxyapatite to hydroxyapatite and hydroxyapatite to collagen is higher in patients with aseptic necrosis of femur’s head than in patients directly after fracture.
It has been observed, that in patients with aseptic necrosis of femur’s head in comparison to patients with osteoporotic fracture, the ratio of alpha helix ordinate structure amount to unordinated structure of statistical bundle is constant.
Based on the results of the necrotic bone tissue of head of femur’s researches, with using Raman’s microspectroscopy method, quantitative and qualitative changes in amount of mineral and organic components in the structure of intracellular spongy bone tissue can be observed.
Conclusions. Further researches can help to exclude of confirm the ischeamic theory of coxarthrosis. This method can be used in recognizing and treatment of aseptic necrosis of the femoral head.
