Therapies of chronic diseases should be efficacious, convenient for the patient and devoid of side effects. In daily practice, the risk of serious outcomes and the preference of patients as well as the cost of the interventions should also be considered. The pathophysiological basis of osteoporosis provides the rationale for the use of interventions that either reduce bone resorption and turnover or stimulate bone formation. Several antiresorptive treatments are used in the treatment of osteoporosis but PTH is the only anabolic therapy currently available (1-34 and recently 1-84). Evidence for efficacy and safety from controlled studies has been obtained for up to 10 years for antiresorptives and up to 18 months for PTH, while short-term head-to-head studies with surrogate endpoints have also been performed. Such studies illustrate the different mechanism of action of the two types of interventions but do not allow any conclusions about any potential differences in antifracture efficacy. These considerations are reflected in recommendations of several regulatory authorities. It is also frequently assumed that antiresorptives should be given mainly to patients with high bone turnover while anabolics should be reserved for patients with low bone turnover. However, analyses of the results of trials with bisphosphonates and PTH 1-34 indicated that the antifracture efficacy of these agents is independent of prevalent rates of bone turnover. Further analysis of the pharmacodynamic responses to these treatments, reveal distinct patterns with attainment or not of steady-states that provide the basis for the design of regimens with the use of both types of therapies, in some patients at least. Such therapeutic approaches need to be explored further and their efficacy in reducing fracture risk, their safety as well as their cost-effectiveness need to be evaluated.

Terapie chorób przewlekłych powinny być skuteczne, wygodne w stosowaniu dla pacjenta i pozbawione działań ubocznych. W codziennej praktyce ryzyko poważnych działań niepożądanych i preferencje pacjenta, jak również koszty leczenia powinny być brane pod uwagę. Patofizjologiczne podstawy osteoporozy dostarczają racjonalnych przesłanek dla zastosowania takich interwencji terapeutycznych, które albo zmniejszają resorpcję kości i obrót kostny, albo stymulują kościotworzenie.
W leczeniu osteoporozy stosowanych jest wiele leków antyresorpcyjnych, ale PTH jest obecnie jedyną, dostępną terapią anaboliczną (1-34 i niedawno 1-84).
Dowody na skuteczność i bezpieczeństwo leków antyresorpcyjnych z kontrolowanych badań uzyskiwane są do 10 lat, a dla PTH do 18 miesięcy , chociaż krótkotrwąjace badania head-to-head z zastępczymi punktami końcowymi także zostały przeprowadzone.
Takie badania obrazują różny mechanizm działania dwóch typów interwencji leczniczej, ale nie pozwalają na żadne wnioski dotyczące potencjalnych różnic w skuteczności przeciwzłamaniowej.
Rozważania te znajdują swoje odbicie w rekomendacjach różnych organów decydujących.
Przypuszcza się też często, że leki antyresorpcyjne powinny być podawane głównie pacjentom z wysokim obrotem kostnym, podczas gdy leki anaboliczne powinny zostać zarezerwowane dla chorych z niskim obrotem kostnym.
Jednakże, analizy wyników przeprowadzonych badań z bisfosfonianami i PTH 1-34 wskazują, ze skuteczność antyzłamaniowa tych substancji jest niezależna od obecnego tempa metabolizmu kostnego obrotu kostnego.
Dalsza analiza farmakodynamicznej odpowiedzi na to leczenie, ujawnia różne modele osiągania lub nieosiągania stanu równowagi dynamicznej, co stanowi podstawę dla tworzenia trybu postępowania z użyciem obu rodzajów terapii, przynajmniej u niektórych pacjentów.
Takie podejście terapeutyczne powinno być dalej badane oraz dalej winna być oszacowana jego skuteczność w redukcji ryzyka złamania, jego bezpieczeństwo jak też kosztoefektywność.
