Wstęp: Osteoprotegeryna (OPG) jako jeden ze związków systemu RANKL/RANK/OPG ma zdolność hamowania resorpcji kości poprzez wpływ na osteoklastogenezę. W niedoborach estrogenów występuje spadek ekspresji OPG związanej ze zwiększeniem aktywności osteoklastów i w związku z tym z zakłóceniem równowagi remodelingu.
Cel pracy: Ocena związku między stężeniem OPG a gęstością mineralną kości (BMD) u pacjentek z zespołem Turnera (TS) w trakcie leczenia hormonalnego preparatem estroprogestagenowym (EP).
Materiał i metody: Do badań zakwalifikowano 32 pacjentki z zespołem Turnera (TS) w wieku od 16-38 lat, z niskorosłością, bez cech pokwitania i z pierwotnym brakiem miesiączki, dotychczas nieleczone preparatami EP, hormonem wzrostu (GH) i preparatami anabolicznymi. Badania densytometryczne (BMD) w odstępach rocznych wykonano z zastosowaniem densytometru (GE Lunar, Madison, WI, USA). Oznaczono stężenia hormonów (TSH, FSH, LH, E2, T) za pomocą testów RIA KIT firmy Orion Diagnostica. Stężenie OPG oznaczono (OPG(1) – przed leczeniem, OPG(2) – po 6 miesiącach leczenia, OPG(3) – po 2 latach leczenia) metodą ELISA (Biomedica, Austria).
Wyniki: Średni przyrost gęstości mineralnej kości (Δ BMD%) w grupie TS był największy po pierwszym 7,5% (±10,9g/cm2; I÷II p<0,0129) i drugim 6,6% (±9,5g/cm2; II÷III p<0,0076) roku leczenia. Po 2 latach leczenia przyrost BMD był nieistotny. Stężenie OPG (1) przed włączeniem leczenia EP wynosiło 4,52 (±0,79)pmol/L, po 6 m-cach OPG(2) 6,47(±1,64) pmol/L (p<0,01), a OPG(3) po 2 latach leczenia EP wynosiło 4,39(±0,78) pmol/L i wartość ta była nieistotna statystycznie w porównaniu do stężenia wyjściowego OPG(1).
Wnioski: Zwiększenie endogennej syntezy OPG jest celem terapeutycznym w obniżonej gęstości mineralnej kości jednakże zależność ta występuje tylko w określonym czasie leczenia estradiolem.

THE INFLUENCE OF 17-BETA-OESTRADIOL ON MODIFICATION OF THE RANK-L/RANK/OPG SYSTEM IN PATIENTS WITH THE TURNER’S SYNDROME

The introduction: Osteoprotegerin (OPG) as one of the components of the RANK-L/RANK/OPG system has ability of inibition of the bones resorption across the influence on osteoclastogenesis. In hypoestrogenism states appear the fall of the expression OPG due to increase of the osteoclasts activity and thereby the disturbance of the remodelling balance.
The aim of the study : The estimation of the relationship between the OPG concentration and the bone mineral density (BMD) in patients with the Turner’s Syndrome (TS) in progress of the hormonal therapy with oestroprogestogen (EP).
Material and methods: To research were classified 32 patients with TS aged 16-38 years, with short stature, without features of the pubescence and with the primary amenorrhea, till now not treated with EP , with growth hormone (GH) and with anabolic preparations. Examined patients determined the natural clinical model of hypoestrogenism.The densitometric examinations (BMD) at intervals one year′s were performed with the use of densitometer (GE Lunar, Madison, WI, U.S.A.). The concentrations of hormones (TSH, FSH, LH, E2, T) were determined by means of RIA kits the Orion Diagnostica firm.. The OPG concentrations were examined before the treatment – OPG(1), after 6 months of the treatment – OPG(2) and after 2 years of the treatment – OPG(3) with the ELISA method (Biomedica, Austria).
Results: The average increase of the mineral bones density (ΔBMD%) in the TS group was the greatest after first 7,5% (±10,9g/cm; I÷II p<0,0129) and second 6,6% (±9,5g/cm; II÷III p<0,0076) year of the treatment. After 2 years of the treatment the increase BMD was insignificant. The concentration OPG (1) before the inclusion of the treatment EP carried out 4,52 (±0,79)pmol/L, after 6 months OPG(2) 6,47(±1,64) pmol/L (p<0,01), and OPG(3) after 2 years of treatment with EP carried out 4,39(±0,78) pmol/L and this value was insignificant statistically in comparison to the primary concentration OPG(1).
Conclusions: The inrease of the endogenic synthesis OPG is a therapeutic aim in the lowered mineral bones density, however after 2 years of the treatment with oestroprogestogens do not grow up the mineral bones density and the concentration of osteoprotegerin returns to the concentration from before the treatment.
Presumably the influence of oestrogens on the increase of the osteoprotegerin expression is limited.
