Wstęp
Konieczność pomocy ludziom cierpiącym z powodu upośledzenia sprawności stawu biodrowego i kolanowego przyczyniła się do podjęcia wielu prób rozwiązań biologicznych i biomechanicznych na przestrzeni lat. Próby ty dotyczyły początkowo postępowania objawowego – przeciwbólowego, następnie przyczynowego – osteotomie, wreszcie biomechanicznego – endoprotezy z różnym skutkiem. Nauka ortopedyczna wiele osiągnęła w zakresie przywracania chorym sprawności, a wespół z biomechaniką uzyskano możliwość mobilności dużych stawów i powrotu chorych do pracy.
Wskazania a trwałość protez
Doraźne i odroczone wyniki przywracania sprawności zniszczonych stawów stanowi zagadnienie trudne, wynikające z możliwości doboru typów rozwiązań protezoplastyk do biologicznego podłoża zmian.
Wskazania a realloplastyki
Według piśmiennictwa, wnikliwa analiza przyczyn obluzowań protez i stopnia zniszczenia tkanki kostnej była podstawą modyfikacji dotychczasowej filozofii protezoplastyk w kierunku pozostawiania możliwie znacznej masy kości chorego.
Wskazania z pozostawieniem zdrowych elementów stawu
Minimalizacja destrukcji operacyjnej zdrowej tkanki kostnej chorego dotyczy wdrożenia nowoczesnych protezoplastyk połowiczych stawu biodrowego w sytuacji prawidłowej chrząstki stawowej panewki stawu biodrowego, czy jednoprzedziałowych protez stawu kolanowego,
Wskazania a masyw kości udowej
Wspomniane elementy stanowią podstawę prezentacji zagadnienia wskazań do totalnych alloplastyk stawu biodrowego i kolanowego.

Introduction
The necessity of the help contributed people sufferings because of the handicap of the efficiency of the hips and knees to undertaking of many the tests of biological solutions and biomechanical, on the space of years. The test you they related to the symptomatical conduct initially – analgesic, then causal – osteotomy, at last biomechanical – replacements with the various result. The orthopaedic science reached many in the of restoring ill range of efficiency, and together with the biomechanics gets the possibility of the mobility of big joints and return ills to the work. The applied since the summers replacements of the hips and knee joint still are the object of scientific and clinical discussions in the range of their biology and biomechanics, according to the literature.
Pathology of the tissue
Pathology of the destruction of the hips and knee joints is extremely diverse: inflammable, traumatic or the degenerative different types. Different every of pathology administer the right biology, what makes up one from the difficulty of the selection of the methods of the reconstruction of worn out joints. The only mechanical conduct – grafting artificial limb with the omission of biological elements authors are careful for not quite proper.
The quality of the osseous tissue is the next controversial element of types replacements of the hips and knee joints in the exchanged groups of pathology what has the essential meaning in the distant results of the treatment.
Indications and the durability of replacements of joint
Temporary and the postponed results of restoring the efficiency of worn out joints, resulting from the possibility of the selection of the types of solutions replacement to the biological basis of changes.
The durability of the grafted artificial limb of the hips joints is the challenge for the present orthopedic. Known he is that she results both from the biological conditions of joints and biomechanics and the biotechnology of artificial limb. Optimization of these three groups of questions is difficult at present and makes up the basis of more far discussions in the literature and the need of investigations.
Indications and realloplasty
Various reports of the literature on the considerable percentage of the necessity revision replacement of the hips or knee joints in the various time from primary operation. Realloplasty make up the interpretation of the primitive workshop, biological imperfection, biomechanical or biotechnological. According to the literature, the thorough analysis of causes the loosening of prosthesis and the degree of the destruction of the osseous tissue was basis of modification of hitherto existing philosophy replacement in direction of leaving possibly considerable mass of bone.
Indication with leaving the healthy elements of the joint
Minimization of the destruction operation of the healthy osseous tissue of the ill relates use modern hemiarthroplasty of hips joint in the situation of the healthy cartilage of the acetabulum hip joint if the monopartial of the knee prosthesis.
Indication and the massif of the femoral bone
Leaving the local tissue of the osseous femoral bone in the procedure of the implantation of the prosthesis, then indication to next solutions, as the tests of biological prosthesis. The short stims of various types are one of them. The next generation of biological prosthesis there are capoplasty various types, leaving not only massif of the closer section of the femoral bone is whose foundation, but conditions biomechanical of the hip joint.
Remembered elements make up the basis of the introduction of the question of indications to the total replacement of the hip and knee joint.
